Default Occasional violent shacking in my 95 Jeep Cherokee 2WD Sport, 145,000miles?

This does not occur all the time, but sometimes when I acclerate my jeep starts vibrating, shaking, and jerking like it is going to fall apart. It sounds horrible, like a machine gun. This usually happens up to 45 miles an hour. If I pull over and shut it down for a minute, it will stop. It happens the most when it is cold and damp out (but this could just be a coincidence) or going up a large hill, and it is about to change gears. What could is possibly be, worst case senerio, and cost. Is it something I can do myself to fix?

I am very worried, I am a single mom with very limited income. If I loose my car, I loose my job, and I loose everything.
