Ok so for my computer class we have to make a tourism website and we need to have a form on it. I don't know the codes or how to do it and I can't find a site that explains it simply. I need a form that someone can input their name, address, email, phone number, and then have some checkboxes where they can check what islands they want more information on. Here's a link to my website so you can get a feel for what I need and what I'm talking about. It needs to be a real working form, so that their responses get directed to me via email.
